Along with our colleagues from Rochford District Primary Care Network, we will be hosting a FREE event for people who have been diagnosed with hypertension (high blood pressure) and/or diabetes, to offer help, advice and support to manage their condition.
We will be joined by representatives from the National Diabetes Prevention Programme, Rayleigh MegaCentre, Rayleigh and Rochford Association of Voluntary Services, Active Essex and Diabetes UK, who will be able to help you find all the tools to stay well.
